To receive these services, patients typically need a referral from a physician. The home health agency will then assess the patient’s needs and develop a personalized care plan. The services are generally provided under the direction of a physician, and the home health agency may coordinate with other healthcare providers to ensure comprehensive care
These services are typically covered by Medicare, Medicaid, and private insurance, but specific eligibility requirements and coverage details vary depending on the insurer and the patient’s condition.
• Medicare, for example, covers home health care if the patient is homebound, under the care of a physician, and in need of skilled care (speech therapy, nursing services, etc.).
We accept several different insurances. Please contact our office for more information, and how you can send referral.
1. Individuals Requiring Assistive Devices or Assistance to Leave the Home: -People who need crutches, canes, wheelchairs, or help from others to move around and leave their home
2. Patients with Dementia, Alzheimer’s, or Memory Impairment: Those experiencing cognitive decline that makes it unsafe to manage their own care or daily activities at home, or who require supervision for safe.
3. Patients Who Have Lost the Use of an Upper Extremity:
• People who have lost the function of an arm or hand may have difficulty with tasks like using a stair handrail or performing daily tasks, and home health care can assist with rehabilitation and safety measures to prevent accidents.
4. Patients with Limited Ambulation Due to Pain or Weakness: - Individuals suffering from pain or weakness (due to conditions like arthritis, injury, or surgery)
5. Patients with Heart or Respiratory Disease: - People with conditions such as heart failure, COPD, or other chronic respiratory or cardiovascular diseases may need home health care
6. People for Whom Outings May Be Medically Contraindicated: - Those with severe medical conditions where leaving the home could worsen their health (for example, post-surgery recovery, infectious diseases, or severe weakness)
In general, home health care is appropriate for individuals who need medical supervision, therapy, or assistance with daily tasks but can remain in the comfort and safety of their own homes instead of seeking care in a facility or hospital.
