Medical Social Services Coordination
. FHC coordinates Medical Social Work services through patients Insurances Resources & established community partnerships to support patients and families as part of the overall plan of care.
. When psycho-social needs are identified, our clinical team facilitates referrals to licensed Medical Social Workers to assist with emotional support, resource access, & care planning.
Services may include:
- Assessment of psycho-social needs impacting health and safety.
- Support with coping, stress, and emotional concerns related to illness.
- Assistance with accessing community resources and support programs.
- Guidance with long-term planning and care decisions.
Medical Social Work services are provided by external community professionals and are coordinated as needed to ensure patients receive comprehensive, patient-centered care.
